From April 26 through 30, a moist and unstable weather pattern affected Puerto Rico and the U.S. Virgin Islands. The episode began as deep tropical moisture moved into the region from the southeast, interacting with deep-layer troughs and supportive jet dynamics aloft. This synoptic setup led to daily rounds of scattered to numerous showers and isolated thunderstorms, especially during the afternoons across the interior and western portions of Puerto Rico.

The 911 call center reported flooding in an underground parking lot next to the Department of the Treasury in the Covadonga area. No individuals were reported stranded.
The 911 call center reported that urban floodwaters have stranded 15 people inside the National Lumber and Hardware Store on Antonio J. Fas Alzamora Road (PR-100) in Cabo Rojo.
Broadcast media posted a video of water from a nearby creek flowing over PR-311 next to Dixon's Balcony Restaurant. However, the road appeared to be passable.
Emergency managers reported that floodwaters from R o Blanco impacted portions of PR-191 and PR-3, near Exit 22 of highway PR-53. They closed the affected road as it was impassable.
The county reported a few landslides along road PR-6103.
Emergency managers reported that waters from Quebrada Maracuto, a tributary of the R o Grande de Lo za, overflowed the Rosa lvarez Bridge along PR-853 in Barrio Barrazas, Carolina. This portion of the road was impassable, affecting the surrounding communities of Camino Los Cuba and Camino Rosa Alvarez.
The 911 call center reported that a residence was affected by floodwaters from a small creek, a tributary of the R o de la Plata, along PR-14 past Iglesia del Faro and Colmado El Fant stico, near kilometer 58.7. No one was stranded.
Broadcast media, shared through social media, showed a video of a creek overflowing and flooding PR-102, with floodwaters visibly affecting the Mangia Italiano restaurant. Although the area was inundated, cars were able to cross the flooded roadway slowly, and no vehicles appeared to be stranded near the restaurant.
The 911 call center reported that two people were trapped inside a vehicle due to floodwaters from the R o Blanco along PR-31, near Exit 22 of highway PR-53. Emergency managers successfully rescued both individuals.
The Municipal Office of Emergency Management in Fajardo reported that PR-194 was impassable due to flash flooding. The Rio Fajardo overflowed its banks, inundating portions of PR-194 between its intersection with PR-3 and Calle Progreso, extending to the intersection with Calle Colon.

Mudslide at Road PR-811 (Camino de los Beefsteaks) near Melendez Pizza.
A landslide was reported by the 911 Call Center, affecting portions of Road PR-615 in Barrio Pozas, Ciales, near kilometer 3.8, close to a bridge and a local body shop. The landslide rendered the road impassable and caused damage to nearby communication and power lines.
The 911 Call Center reported that individuals were stranded in a Kia vehicle due to flash flooding from nearby streams along Camino Los Navarro in Barrio Quebrada Arenas, near Road PR-1 in San Juan. The floodwaters inundated the roadway, creating hazardous conditions and prompting a rescue response.
Emergency managers reported flooding in low-lying areas along Camino Tabonucal in San Juan. The flooding was associated with runoff accumulation affecting the roadway and the surrounding terrain.
Emergency managers reported that Road 1 was impassable near the Ceramic Home Center due to flooding, resulting in the temporary closure of the affected section.

A landslide was reported along Luis A. Ferr Highway (PR-52) near kilometer 53.2, shortly after the Monumento al J baro and near the Cayey Salinas municipal boundary. Although debris was present, the highway remained open and was not reported as impassable.
Emergency managers reported a landslide affecting Luis Antonio Wito Morales Crespo Street (PR-139) near Hacienda Carmelita, approximately 5 miles south of Jayuya. The report did not indicate whether the road was blocked or remained passable.

A landslide was reported on Road PR-369 at kilometer 1.4 in Barrio Cerro Gordo, Sabana Grande. The slide occurred near a local basketball court and close to the intersection with Los Rosados Way. Although debris was present, the road was not reported as impassable.
